If the English version of the application is used and scanning facility is easily available, please scan the application and email it to .

If scanning facility is not easily available, please type up the answers to all the questions on the form, attach a photograph, and email it to .

Applications directly from parents are highly discouraged.

Please keep at least one copy of the application form with you for your own records.

The deadline to submit new applications is February 22, 2008 (Falgun 22, 2064). CE will not consider incomplete applications. An application is complete when all the information on the application (including signatures and the picture on the form) is received.
